New Patients

Thank you for choosing Florence Pediatric Dentistry for your child's dental needs. Please download the New Patient Forms and fill them out prior to your child's first appointment. If your child was previously seen in another office, please fill out the Records Release form so that we may obtain a copy of your child's previous dental records. A parent or legal guardian must be present at the initial appointment. Please bring all medications and any information regarding your child's health history to that appointment as well. Also please bring copies of dental and medical insurance cards

When do I see the dentist?

The American Academy of Pediatric Dentistry (AAPD) recommends that a child be seen for a first dental visit by age one.

How do I prevent cavities?

Brush your child’s teeth for him twice a day. Around age 8, your child may be able to begin to brush without assistance, but supervision should continue into adolescence.
